Wound Care Coordinator - RN Position

Genesis Healthcare LLC is seeking a skilled Wound Care Coordinator - RN to join our team. As a key member of our nursing center, you will play a vital role in leading our skin health and wound management program.

Key Responsibilities:

* Lead and integrate a fully developed Skin Health and Wound management program.
* Collaborate with nursing center management and staff, clinical leadership, Wound Specialists, vendors, and others to effectively communicate with patients and their families regarding the prevention and management of skin and wounds.
* Support central supply as the clinical expert for skin, wound, and incontinent products and provide oversight for durable medical equipment related to the prevention and management of wounds.

Requirements:

* Must possess a RN license (MPT/DPT with leadership review/approval) *Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire *Commitment to attend monthly support calls is required. *Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire (For those hired with wound certification, completion of a fast-track hybrid program for regulatory wound care requirements will be provided) *Clinical experience in a skilled nursing setting is strongly preferred. *Discharge planning experience is strongly preferred. *Knowledge and experience with clinical charting, incident reporting, and investigation response is required. *Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team. *Experience in team building and demonstrating respect for the interprofessional team in wound management. *Must have knowledge of national wound guidelines and wound products. *Knowledge of nursing home regulations and survey process is required. *Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required.
