Job Summary

Esperta Health is seeking a highly skilled and experienced Nurse Practitioner to join our team as an In-Home Wound Management Specialist. As a key member of our team, you will be responsible for providing expert wound care to patients in the comfort of their own homes.

As a Wound Care Specialist, you will work closely with our team to develop and implement individualized treatment plans for patients with chronic wounds. You will be responsible for assessing patient needs, providing wound care and management, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

We are looking for a dedicated and compassionate healthcare professional who is passionate about delivering high-quality patient care and making a meaningful difference in the lives of our patients.

Responsibilities
  • Provide expert wound care and management to patients in their homes
  • Develop and implement individualized treatment plans for patients with chronic wounds
  • Assess patient needs and provide wound care and management
  • Collaborate with other healthcare professionals to ensure optimal patient outcomes
  • Document patient care and treatment plans in electronic health records
Requirements
  • Current, unencumbered nursing license in the state of practice
  • Reliable internet access and cell phone supplied by company
  • Willingness to participate in a call schedule
  • Reliable transportation and an unrestricted driver's license
Qualifications/Education
  • Certified through ANCC (American Nurses Credentialing Center) or AANP as a Nurse Practitioner
  • WOCN (Wound, Ostomy and Continence Nurses) certification or other company-approved wound care certification required
  • U.S. Citizenship
  • Currently licensed as a professional nurse and advanced practice nurse in good standing
  • Proficient in English
  • Meets physical standards
  • BLS certification
Experience
  • Minimum of 3 years' experience treating chronic wounds in a hospital, outpatient, or post-acute setting
  • Direct patient care experience
  • Electronic patient records proficiency
Skills
  • Superior communication skills, both oral and written
  • Exemplary understanding of evidence-based wound care, ability to educate colleagues, patients, and caregivers to aid positive outcomes
  • Ability to work independently and collaborate with other healthcare members
  • Ability to build professional relationships with peers and colleagues and share knowledge through coaching and mentoring
  • Proficient computer skills including the ability to use Microsoft Products and Google documents
  • Ability to think critically and multitask
  • Demonstrated accountability
