Wound Care Specialist

Job SummaryWe're seeking a skilled Wound Care Specialist to join our team. As a key member of our in-home wound management program, you will be responsible for providing expert wound care services to patients in their homes. This is a challenging and rewarding role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.
Key Responsibilities- Provide expert wound care services to patients in their homes, including assessment, treatment, and education.
- Develop and implement individualized wound care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Conduct regular patient assessments to monitor wound healing progress and adjust treatment plans as needed.
- Document patient information and treatment plans in a timely and accurate manner.
- Current, unencumbered nursing license in the state of practice.
- Reliable internet access, cell phone supplied by company.
- Willingness to participate in a call schedule.
- Reliable transportation and an unrestricted driver's license.
- Certified through ANCC (American Nurses Credentialing Center) or AANP as a Nurse Practitioner.
- WOCN (Wound, Ostomy and Continence Nurses) certification or other company-approved wound care certification required.
- U.S. Citizenship.
- Currently licensed as a professional nurse and advanced practice nurse in good standing.
- Proficient in English.
- Meets physical standards.
- BLS certification.
- Minimum of 3 years' experience treating chronic wounds in a hospital, outpatient, or post-acute setting.
- Direct patient care experience.
- Electronic patient records proficiency.
- Superior communication skills, both oral and written.
- Exemplary understanding of evidence-based wound care, ability to educate colleagues, patients, and caregivers to aid positive outcomes.
- Ability to work independently and collaborate with other healthcare members.
- Ability to build professional relationships with peers and colleagues and share knowledge through coaching and mentoring.
- Proficient computer skills including the ability to use Microsoft Products and Google documents.
- Ability to think critically and multitask.
- Demonstrated accountability.
