Job Summary

We are seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for planning, organizing, and directing hospice care for patients and their families. As a Case Manager, you will work closely with the interdisciplinary team to provide comprehensive care and support to patients in their homes and communities.

Key Responsibilities
  • Conduct initial and ongoing assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
  • Collaborate with the interdisciplinary team to coordinate care and services, including home health aides, social workers, and chaplains.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
Requirements
  • Graduate of an accredited school of nursing with a current license to practice in the state.
  • One to two years of recent acute care experience in an institutional setting, with a preference for experience in home care.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
