Position Overview
Role: Hospice RN Case Manager
Schedule:

  • Monday - Friday
  • 8 AM - 4:30 PM
  • 64 hours per pay period
  • Support for the Baxter Hospice Division

Key Responsibilities
• Deliver clinical care and case management for hospice patients and their families.
• Supervise and guide the Hospice Aide.
• Maintain regular communication with the patient's physician, interdisciplinary group (IDG), and family regarding the care plan, pain management, and any arising concerns.
• Collaborate in the development and execution of personalized care plans, ensuring safety and quality in compliance with federal, state, and organizational regulations.

Qualifications
• Proficient in reading, writing, and speaking English to effectively communicate with staff, residents, and visitors. Must complete a state-required background study.
• Ability to communicate verbally and in writing as required by the position. Must possess a valid driver's license and insurance to travel to clients' homes. Adequate computer skills are essential.
• Current Registered Nurse licensure in Minnesota.
• Current BCLS certification.


Additional Responsibilities
• Conduct comprehensive assessments during patient visits, ensuring accurate documentation.
• Assist patients and families in creating individualized care plans that align with their goals.
• Demonstrate knowledge of comfort medications and hospice protocols.
• Provide clinical education and ongoing support to patients and caregivers.
• Supervise hospice aides at least bi-weekly and participate in bereavement planning for families.


Interdisciplinary Team Collaboration
• Engage in continuous communication with the care team and IDG members.
• Appropriately refer patients to ancillary services and participate in care conferences to discuss care plans and goals.


Customer Service Orientation
• Prioritize customer experience in all actions, understanding that customers include patients, families, and community partners.
• Foster positive relationships and provide prompt resolutions to concerns.


Interpersonal Skills and Leadership
• Adapt communication styles to suit various audiences and situations.
• Practice accountability in daily tasks and contribute to a positive workplace culture.
