Key Responsibilities
  • Participate in the care planning process, collaborating with Care Managers to assess, set goals, and develop interventions for our members.
  • Conduct telephonic and face-to-face interactions, including home visits, to provide comprehensive care management services.
  • Focus on quality of care, compliance, outcomes, and cost reduction, while developing strategies to coordinate and integrate post-acute and long-term care services.
  • Perform initial and periodic assessments of members enrolled in our Long-Term Care Program and/or care programs.
  • Apply social work concepts, principles, and strategies to address the social determinants of health needs in members' individualized care plans.
Requirements
  • Minimum of 2+ years of experience in medical social work or case management.
  • Active and unrestricted LMSW or LCSW licensure in the state of Florida.
  • Ability to travel up to 50-75% of the time in the Miami-Dade region, with reliable transportation and mileage reimbursement.
Preferred Qualifications
  • Bilingual (Spanish/Creole speaking).
