Job Summary

We are seeking a skilled and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in their homes, helping them to recover from illness, injury, or surgery, and maintain their independence.

Key Responsibilities:

  • Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC)
  • Assist patients with self-administered medications, take vital signs, and report any changes or concerns to the supervising nurse
  • Help patients maintain good personal hygiene, including bathing, grooming, and skin care
  • Assist with feeding patients, communicate basic principles of nutrition, and observe and record food and fluid intake when necessary
  • Provide necessary skills to safely assist patients with mobility, exercises, positioning/turning, transfers, and ambulation per Plan of Care and CenterWell Home Health policy
  • Document interactions with patients, caregivers, doctors, and other staff members appropriately, legibly, thoroughly, and in the amount of time allowed

Requirements:

  • High school diploma or equivalent
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
  • Must meet applicable state certification requirements
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency
  • At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
