Key Responsibilities:
  • Collaborate with licensed clinicians to develop and implement individualized treatment plans.
  • Provide linkage and interventions to support clients in achieving their goals.
  • Develop and maintain relationships with community resources and service providers.
  • Monitor and evaluate client progress, making adjustments to treatment plans as needed.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.

Requirements:
  • High school diploma or equivalent, plus three or more years of relevant experience or a Bachelor's or Master's degree in a related field.
  • Valid driver's license, car insurance, and reliable transportation.
  • Ability to travel throughout assigned service area.
