Job Summary:

As a Case Manager at Behavioral Health Group, you will play a vital role in helping patients achieve their social, educational, and occupational goals. You will provide referrals, coordinate care, and adhere to all regulatory requirements, reporting to the Counselor Supervisor or Program Director.

Key Responsibilities:
  • Conduct regular drug abuse case management sessions to support patients in their recovery journey.
  • Orient patients to treatment program operations and procedures, ensuring a smooth transition into care.
  • Collect and assess social, educational, and vocational information to develop comprehensive treatment plans.
  • Make appropriate referrals and perform follow-ups to ensure patients receive the necessary support.
  • Assist in implementing new programs and advise on caseload management to optimize patient outcomes.
  • Provide crisis intervention and inter-agency documentation to ensure timely and effective support.
  • Coordinate care with community providers to ensure a seamless continuum of care.
  • Achieve annual goals and develop knowledge of addiction and related regulations to stay up-to-date in the field.
  • Participate in in-service trainings and attend relevant conferences to enhance professional development.
  • Adhere to all federal, state, and local regulations and accrediting agency requirements to maintain compliance.
  • Participate in community and public relations activities to promote the organization's mission and values.
Requirements:
  • Bachelor's degree in a related Human Services field.
  • Pursuing licensure as a Certified Alcohol and Drug Counselor (CADC) is preferred.
  • Compliance with federal and state certification and licensure regulations.
  • Knowledge of addiction, treatment methods, and current trends in opioid maintenance treatment is preferred.
