Palliative Care Nurse Practitioner Opportunity

Health Link Home Health Agency and Hospice is a renowned provider of comprehensive home health services in the greater San Francisco Bay Area. We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Nurse Practitioner for our Palliative Care Program. As an organization, we value the importance of compassion and empathy in delivering healthcare services.

Job Summary

The Nurse Practitioner provides routine care and management of patients with acute and chronic complaints, in collaboration with the supervising physician and through implementation of standardized procedures.

The Nurse Practitioner reports to the Director of Nursing Support Services and is responsible for working in collaboration with a physician, and other palliative care team members, in an active practice to deliver palliative care and related healthcare services to patients and families.

Responsibilities
  • Oversee and manage day-to-day operations of the Program
  • Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel
  • Develop, implement, and evaluate the orientation program for new organization personnel
  • Assist clinical team members to develop/enhance their skills and techniques in providing exceptional patient care
  • Assist in the development of organization goals
  • Develop, recommend, and administer organization policies and procedures
  • Provide palliative care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families, and other healthcare practitioners
Requirements
  • Education and/or work experience in palliative care preferred
  • Must have excellent assessment, diagnosis, and treatment skills, strong communication skills, and be self-directed, motivated, and highly organized
  • As required for state of CA license as an RN and Certified Nurse Practitioner
