Annual Salary: $88,000 to $173,200 (Washington Residents Only)

Job Summary:

UnitedHealth Group is seeking a Population Health Specialist to provide expert care to patients across the state. The successful candidate will focus on improving care models, educating clinicians, and promoting quality healthcare through chronic condition reviews and risk adjustment best practices.

Main Duties:

  • Improving Care Models: Develop and implement effective care models that improve patient outcomes and reduce costs.
  • Educating Clinicians: Provide training and education to clinicians on risk adjustment best practices and CMS's HCC-based model.
  • Promoting Quality Healthcare: Promote best practices in HEDIS measures of quality, dementia and caregiver support programs, and transitional care interventions.

Key Qualifications:

  • Education: An Advanced Registered Nurse Practitioner degree from an accredited institution
  • Licensure: A current and unrestricted state license to practice medicine as a Nurse Practitioner or ability to obtain by start date
  • Skills: Strong communication and leadership skills, proficiency in Microsoft Office applications, and ability to travel occasionally within licensed states
