Job Overview

We are seeking a compassionate and skilled Home Health Care Specialist to join our team at GRACEHOPE HOME CARE LLC. As a vital member of our care team, you will play a crucial role in providing exceptional support to patients with physical and mental challenges.

About the Role:

  • Provide personalized care and assistance to patients with daily living activities, including medication reminders, mobility, bathing, grooming, light housework, and emotional support.
  • Collaborate with patients and their families to develop comprehensive care plans tailored to their unique needs.
  • Maintain accurate records of patient progress, incidents, and concerns.
  • Conduct regular assessments to ensure patients receive optimal care and make recommendations for improvements.

Requirements and Qualifications:

  • Patient Care Assistant (PCA) certification or Home Health Aide (HHA) licensure.
  • CPR certification and valid driver's license.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management abilities.
  • Ability to work independently and as part of a team.

Compensation and Benefits:

Our competitive salary range for this position is $45,000 - $55,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
