Job Summary

The Housing Focused Clinical Case Manager will work with shelter guests to secure safe alternative housing placements or permanent housing, linking them with needed supports to sustain the housing placement.

Key Responsibilities
  • Engage and build strong working alliances with individuals experiencing a housing crisis.
  • Assist residents in a manner that demonstrates respect, honesty, non-judgment, and empathy.
  • Complete intakes for new guests, including assessments, case plans, and releases of information.
  • Develop individualized housing plans with each participant.
  • Provide linkages to community resources for sustainability, including medical, mental health, substance abuse, child care, and transportation services.
  • Maintain documentation of trainings, budgets, housing contacts, and other pertinent information.
  • Facilitate weekly community meetings with shelter residents.
Requirements
  • Bachelor's degree in Psychology, Mental Health, or a related field, or equivalent experience.
  • Minimum of three years of experience providing service to persons experiencing homelessness, mental health, or addiction issues.
  • Excellent written and oral communication skills.
  • Licensed or licensed eligible in Social Work, SUD, Psychology, or Counseling is preferred.
Work Environment

The work environment involves risks or discomforts, including exposure to aggressive human behavior. The position requires physical effort, including cleaning, pushing, pulling, and moving objects.
