We are seeking a highly skilled and compassionate Clinical Coordinator to join our team at DaVita. As a Clinical Coordinator, you will play a vital role in delivering exceptional patient care and services in our outpatient dialysis center.

About the Role

As a Clinical Coordinator, you will be responsible for coordinating ESRD patient care and services, ensuring the highest level of care is delivered to our patients. You will work closely with our clinical team to ensure patient safety, comfort, and well-being.

Responsibilities
  • Coordinate ESRD patient care and services, including trend lab values, tracking metrics on vital signs, weight, and outcome management.
  • Ensure the delivery of exceptional patient care by ensuring patient safety, comfort, and well-being.
  • Collaborate with the clinical team to develop and implement patient care plans.
  • Provide training and support to new staff members.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
