Patient Services Coordinator

At CenterWell Home Health, we are dedicated to providing personalized, comprehensive home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization. As a Patient Services Coordinator, you will play a vital role in our care teams, working closely with nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Manage patient schedules, ensuring timely and efficient care coordination and continuity of care.
- Communicate effectively with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and ensure seamless transitions.
- Assist with day-to-day office and staff management, including scheduling, reporting, and data entry.
- Initiate infection control forms as needed and ensure compliance with regulatory requirements.
- Provide exceptional customer service, responding to patient and family inquiries, and addressing concerns in a timely and professional manner.
- Collaborate with the care team to develop and implement individualized care plans, ensuring patient-centered care and optimal outcomes.
- Monitor and report on patient progress, identifying areas for improvement and implementing changes as needed.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- 1+ year of home health experience, with a strong understanding of care coordination and patient-centered care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple assignments.
- Coding certification and/or prior packet review/QI experience preferred.
- Valid state driver's license and automobile liability insurance required.
At CenterWell Home Health, we offer a competitive compensation package, including a salary range of $37,200 - $51,200 per year, depending on experience and qualifications. We also offer a comprehensive benefits package, including medical, dental, and vision benefits, 401(k) retirement savings plan, time off, and opportunities for professional growth and development.
