The Palliative Care program is dedicated to providing specialized medical assistance for individuals facing serious health challenges. We are seeking a skilled Nurse Practitioner to join our exceptional Palliative Care team.

POSITION SUMMARY

This role involves collaboration with the Palliative Care Physician, Director, and a multidisciplinary team. The Nurse Practitioner will deliver clinical expertise, uphold care standards, and offer consultation in patient care delivery. Responsibilities encompass patient consultations, care provision, and clinical oversight, working closely with physicians and clinical staff during patient interactions.

KEY RESPONSIBILITIES

Patient Care:

  • Collaborate with the Palliative Care Director and team members to ensure seamless daily operations, compliance with agency and regulatory standards, and the delivery of high-quality patient care.
  • Contribute significantly to the formulation of comprehensive care plans by identifying suffering symptoms and managing holistic patient and family-centered care, setting realistic care goals in partnership with primary care physicians and local healthcare facilities.
  • Provide clinical guidance and oversight in conjunction with other healthcare providers.
  • Conduct patient visits for both existing and new patients under physician supervision.
  • Facilitate effective multidisciplinary communication and foster collaborative relationships within a customer-focused environment.
  • Participate in the orientation and training of new staff, serving as a resource for team members.
  • Offer backup and coverage responsibilities for physicians as necessary.
  • Prescribe medications (non-narcotic) within the scope of delegation and licensure.
  • Order necessary treatments and durable medical equipment as required.
  • Consult with physicians or their designees as needed and collaborate with other healthcare professionals.
  • Maintain accurate patient records and documentation to support sound medical practices.
  • Manage patient discharges as conditions dictate or upon patient request, completing discharge summaries.
  • Adhere to applicable laws and regulations regarding Collaborative Agreements.
  • Engage in the development of the Plan of Care for all patients in collaboration with the interdisciplinary team.
  • Reassess patients' physical, psychosocial, and spiritual needs, notifying the appropriate team members as necessary.
  • Refer and consult with other disciplines based on patient and family needs and the Plan of Care.
  • Educate and support patients and their families regarding diagnoses and care plans.
  • Deliver cost-effective care within established fiscal parameters.
  • Communicate changes in patient conditions and needs to the physician and interdisciplinary team members.
  • Provide counseling to patients and families concerning physical and emotional needs.
  • Assess the appropriate level of care required to meet patient needs.
  • Maintain professional relationships and effective communication with nursing home staff.
  • Offer education and mentorship to staff as requested.
  • Conduct supervisory visits as required.

Administrative Responsibilities:

Scheduling:

  • Assist the Branch Office Manager with scheduling duties.
  1. Ensure availability for administrative coordination at least two weeks in advance.
  2. Meet scheduled patient visit expectations.
  3. Follow all documented scheduling guidelines.
  • Complete documentation for each visit, including assessment tools, and submit within seven days.
  • Ensure necessary referrals are made based on assessment results and communicate details securely.
  • Attend interdisciplinary team meetings to discuss patient care and future plans as requested.
  • Comply with departmental and regulatory documentation requirements.
  • Document activities performed and submit for billing processing within the designated timeframe.
