Job Overview

Visiting Nurse Association of Southeast Missouri is seeking a dedicated Speech Language Pathologist to enhance our VNA Home Health team.

ABOUT VNA HOME HEALTH: Our Home Health program offers essential services to individuals requiring specialized care as directed by their healthcare provider, delivered in the comfort of their own homes. The Visiting Nurse Association of Southeast Missouri operates as a non-profit organization committed to community health.

RESPONSIBILITIES:

  • Schedule initial assessments (therapy, nutritional, etc.) within 48 hours of the service request, unless otherwise directed.
  • Conduct initial and ongoing evaluations using the designated forms and software provided by VNA.
  • Collaborate with the supervisor and patient to create a service schedule that aligns with patient needs and physician orders.
  • Notify the VNA supervisor if there are any challenges in delivering the prescribed services.
  • Communicate patient discharge or transfer needs to the Total Care Manager before contacting the physician.
  • Work with the physician to formulate a treatment plan and deliver direct patient services as per the established plan and orders.
  • Regularly review and update the care plan based on continuous assessments.
  • Complete clinical documentation and progress notes in accordance with regulatory requirements.
  • Assist the physician in assessing the patient's health status.

TRAINING: VNA offers a comprehensive 30-day orientation program for new hires, allowing you to shadow experienced Home Health Team Members for practical training. This includes opportunities for direct interaction and questions during patient visits, as well as training on the technology used for patient documentation.

POSITION REQUIREMENTS:

Education: A Bachelor of Science or Master of Science Degree in Speech Therapy from an accredited institution recognized by the Commission of Accreditation for Speech Therapy Education.

Experience: A minimum of one year of experience in a healthcare setting as a Speech Language Pathologist is preferred, with home health experience being an advantage.
