Job Title: Renal Transplant Coordinator

Mass General Brigham is seeking a highly skilled and compassionate Renal Transplant Coordinator to join our team. As a key member of our transplant program, you will play a vital role in ensuring the seamless coordination of care for our patients undergoing renal transplantation.

Responsibilities:
  • Assess, plan, implement, and evaluate the plan of care for post-operative renal transplant patients.
  • Provide in-patient teaching and support to patients and their families, ensuring a smooth transition to home care.
  • Collaborate with the multidisciplinary transplant team to coordinate patient care, including medical management, wound care, and medication administration.
  • Coordinate patient follow-up appointments, including review of lab results and revision of care plans as needed.
  • Facilitate interactions between the transplant team, patients, and families, ensuring clear communication and a positive patient experience.
  • Participate in interdisciplinary meetings to discuss patient care and program development.
Requirements:
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum two years of experience in renal medicine-focused patient care.
  • Minimum two years of experience in abdominal surgery or transplant-focused patient care.
  • Triage experience and ability to educate patients on immunosuppressants, rejection, and infection treatments.
  • Ability to work independently and as part of a team, with excellent communication and problem-solving skills.
