About the Role

Mass General Brigham is seeking a highly skilled and compassionate Nurse Coordinator to join our Renal/Pancreas Transplant team. As a key member of our multidisciplinary team, you will play a vital role in ensuring the seamless care of our patients undergoing renal and pancreas transplants.

Key Responsibilities
  • Coordinate patient care from pre-transplant evaluation to post-transplant follow-up, ensuring timely and effective communication with patients, families, and healthcare providers.
  • Develop and implement individualized care plans, taking into account patients' unique needs and circumstances.
  • Collaborate with transplant nephrologists, surgeons, and other healthcare professionals to ensure comprehensive and coordinated care.
  • Provide education and support to patients and families, addressing their concerns and promoting optimal health outcomes.
  • Participate in interdisciplinary team meetings, contributing to the development and implementation of care plans and policies.
  • Stay up-to-date with the latest research, guidelines, and best practices in renal and pancreas transplantation, applying this knowledge to improve patient care.
Requirements
  • Current registration in Massachusetts as a Registered Nurse (RN).
  • Minimum two years of experience in renal medicine-focused patient care, with a strong background in transplant nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
