Position Overview:


The Care Coordinator plays a vital role in managing all elements of Comprehensive Community Services (CCS) service planning. This includes, but is not limited to, transporting clients as necessary, initiating contact with referred individuals, and establishing a trusting relationship through a strength-based, trauma-informed, recovery-oriented, and culturally sensitive approach.


Within 30 days of the CCS application, the Care Coordinator will develop an Individualized Recovery Plan tailored to the assessed needs of the individual. This involves authorizing psychosocial rehabilitation services related to mental health and substance use, as well as setting measurable goals and tracking progress towards desired outcomes.


The role also includes facilitating reviews of the Individualized Recovery Plan, ensuring updates are made based on the individual's evolving needs or at least every six months.


Additionally, the Care Coordinator is responsible for ensuring that individuals receive timely assessment services, service planning, service delivery, and supportive activities/services.


It is essential that the Individualized Recovery Plan and service delivery for each individual are coordinated, monitored, and structured to assist the individual in achieving the highest possible level of independent functioning.


The Care Coordinator will support individuals in self-advocacy and help them access necessary services, including medical, dental, legal, financial, and housing support.


Compliance with legal, organizational, and contractual requirements, as well as relevant laws and policies, is crucial.


Key Responsibilities:

  1. Prepare, complete, and submit required documentation by established deadlines.
  2. Establish and maintain effective communication and collaboration with all stakeholders.
  3. Implement and adhere to the responsibilities of a mandated reporter regarding suspected abuse.
  4. Attend all required meetings and professional development activities.
  5. Coordinate client intakes.
  6. Perform additional duties as assigned.


Knowledge, Skills, and Abilities:

  • Comprehensive understanding of federal, state, and local laws, codes, and regulations pertaining to services for chronic mental illness.
  • Familiarity with protective payee responsibilities.
  • Knowledge of chronic mental illness, dual diagnosis, and treatment methodologies.
  • Ability to work effectively with adults experiencing mental health diagnoses and developmental disabilities.
  • Strong relationship-building skills with community agencies and organizations.
  • Commitment to maintaining confidentiality.
  • Effective communication skills with clients, staff, service providers, and volunteers.
  • Capability to develop, organize, and implement diverse programs.
  • Ability to follow instructions and work independently or collaboratively.
  • Willingness to attend required training sessions.
  • Possession of a valid driver's license and access to a reliable, insured vehicle.


Educational Requirements:
A Bachelor's degree is required, while a Master's degree is preferred.
