Client Care Specialist, CCS

GENERAL SUMMARY:
Comprehensive Community Services (CCS) is dedicated to providing recovery-oriented behavioral health support for adults facing severe mental health challenges and/or substance use issues. This community-driven initiative emphasizes delivering most services within the clients' environments, ensuring a person-centered approach that utilizes individualized service plans tailored to assist clients in reaching their recovery objectives. Care Coordination is a pivotal role within this framework, where Care Coordinators collaborate with teams of professionals, peer advocates, and natural support systems.
Care Coordinators are instrumental in fostering skills that promote independence, self-assurance, and leadership, empowering individuals to take charge of their lives and fulfill their aspirations.
KEY RESPONSIBILITIES:- Deliver comprehensive care coordination services to clients throughout their engagement with CCS, guiding them on how to effectively leverage both formal and informal community resources.
- Process referrals for eligible clients promptly, initiating assessments to identify strengths and needs.
- Lead team meetings to create care plans that reflect clients' strengths, values, and preferences.
- Support the team in defining measurable outcomes for clients while addressing any systemic barriers that may arise.
- Maintain accurate documentation of client plans, gather outcome data, and ensure all records meet program deadlines.
- Conduct annual Functional Screens to verify client eligibility.
- Manage a caseload of 15-18 clients, tailored to their specific needs.
- Participate in ongoing training and professional development opportunities.
- Comply with St. Charles Youth & Family Services Policies and Procedures.
- Perform additional duties as assigned.
- A Bachelor's degree is required; a Master's degree in a relevant field is preferred.
- Minimum of one year of experience in a mental health-related environment.
- Understanding of individual and family dynamics.
- Proficient in preparing formal reports.
- Exhibit cultural competence and sensitivity towards diverse populations.
- Excellent verbal and written communication skills.
- Possession of a valid driver's license and liability insurance.
- Ability to utilize vehicles for work-related responsibilities in accordance with agency insurance standards.
- Competence in maintaining accurate records and documentation within specified timeframes.
- Familiarity with computer applications is preferred.
- Work will be conducted in a general office setting as well as various community locations and clients' residences.
- Travel is required approximately 70% of the time, including transporting clients using agency vehicles.
