Role Overview:
The Practical Nurse Specialist will deliver clinical oversight and assistance for residents experiencing Alzheimer's/Dementia. This position ensures that all restorative, supportive, and preventative measures are adhered to in compliance with relevant state laws and regulations.
Experience in working with Alzheimer's/Dementia or behavioral health issues, along with a commitment to serving the elderly population, is crucial for this position.

Key Responsibilities:
1. Engage actively in resident evaluations, the creation of service plans, and daily care routines.
2. Oversee and execute the self-administered medication management program for residents.
3. Administer medications and/or treatments to residents per physician directives.
4. Document and communicate care provided and resident observations, including any unusual or significant changes in physical, mental, or behavioral conditions.
5. Provide emotional support, physical comfort, and ensure the ongoing safety of residents.
6. Supervise Care Partners and Home Health Aides in delivering resident care in accordance with State, Federal, and Artis guidelines.
Ensure residents receive the highest quality of service in a compassionate and caring environment that recognizes their individual needs and rights.
Assist in community marketing by conducting tours and participating in public relations events; build relationships with potential resident referral sources.
Assist with resident transitions and relocations, helping residents adjust to their new environment and community personnel.
Qualifications:
Must hold a current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as issued by the State Board of Nursing.
Maintain and fulfill continuing education credits as mandated by state regulations.
Experience in rehabilitative or geriatric nursing is preferred.
