Position Overview

Role: Clinical Social Services Specialist

Objective:

To deliver essential support services that enable each resident to achieve their highest level of emotional and psychological wellness.

Key Responsibilities:

  • Conduct continuous case management, which includes interviews, crisis intervention, bereavement support, psychosocial evaluations, referrals, discharge planning, assessments of suicide risk, and advocacy efforts.
  • Accountable for compiling social histories and updating resident records.
  • Facilitate individual counseling sessions for residents, families, and staff members.
  • Engage in collaborative care planning with interdisciplinary teams and ensure timely documentation of care plans and progress notes in compliance with licensure and professional standards.
  • Complete necessary assessments and maintain documentation for tracking purposes.
  • Assist residents and families in navigating application processes and eligibility for Medicaid and other support programs.
  • Ensure timely completion of required screenings and documentation within residents' medical records.
  • Prepare documentation for submission to the Medical Advisory Committee.
  • Organize family meetings to prepare residents and families for changes in care levels.
  • Conduct clinical audits as directed by management.
  • Participate in regular administrative meetings and committees relevant to the role.
  • Maintain accurate social work records and files for residents.
  • Provide coverage for colleagues as necessary.
  • Collaborate with the social work team and interdisciplinary staff to achieve departmental and organizational objectives.
  • Contribute to the establishment of departmental goals.
  • Engage in ongoing professional development and mandatory training sessions.
  • Stay informed about Medicare, Medicaid, Social Security, and other relevant benefits.
  • Maintain knowledge of the probate court system and community-based resources beneficial to residents.
  • Adhere to established infection control and safety protocols.
  • Report any unsafe conditions or defective equipment to the appropriate departments.
  • Comply with HIPAA and corporate compliance regulations, and be able to explain them as needed.
  • Facilitate educational seminars and training sessions for staff and residents as required.
  • May deliver educational presentations to external groups.
  • Miscellaneous: Maintain a record of reliable attendance and punctuality. Adhere to all organizational policies and safety procedures. Perform other duties as assigned.

Qualifications:

Must possess an Ohio Licensed Social Worker credential, a Bachelor of Social Work (BSW) degree, and demonstrate proficiency in computer skills. A minimum of three years of experience in a geriatric care setting is required.

Physical Requirements:

Ability to navigate the facility independently (on foot or using a wheelchair). Must possess adequate vision and hearing, or utilize assistive devices to perform job functions effectively. Must be capable of communicating in English clearly and understandably.

Ability to sit at a desk and use a computer for extended periods.

Work Environment:

Work will occur in various settings, including resident rooms and communal areas within the facility. The role involves frequent movement throughout the day and may include interruptions. Exposure to infectious materials and diseases is possible.

The office environment may involve shared spaces, limited natural light, and occasional noise, with privacy not always guaranteed.
