St. Luke's Health System is seeking a skilled Social Work Specialist to join our team in Twin Falls, Idaho.

Job Summary

This is an ongoing, full-time position that requires a strong commitment to delivering exceptional patient experiences.

Key Responsibilities
  • Provide intensive onboarding and training to ensure a smooth transition into the role.
  • Work primarily in an outpatient setting, with opportunities to see patients in the hospital, Cancer Institute, skilled nursing facilities, and in-home.
  • Develop and implement effective care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Masters degree in social work from an accredited institution.
  • Licensed Masters Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Idaho.
  • Experience in Palliative Care or Hospice preferred.
