Intermountain Deaconess Children's Services is a leading non-profit organization dedicated to providing support and healing to children, adolescents, and families facing challenges in mental and behavioral health. We are seeking a dedicated Case Manager to facilitate communication and collaboration between clients and various external parties.

Compensation: $18 - $20 per hour

Key Responsibilities:

  1. Act as the primary contact for families and external stakeholders, addressing any issues that may arise during the treatment process.
  2. Provide education to families and stakeholders regarding treatment methodologies, philosophies, and expectations for parental involvement.
  3. In collaboration with the treatment team, document and convey sensitive information in accordance with established policies and timelines.
  4. Ensure all necessary documentation related to services, including consents, releases, and treatment records, is completed in compliance with relevant regulations.
  5. Maintain accurate records of case management activities.
  6. Verify financial and clinical eligibility for services prior to service delivery, assisting families with financial arrangements and referrals to funding resources.
  7. Coordinate with the Accounting/Payroll department to ensure timely billing for case management services.
  8. Lead and oversee activities related to referrals, admissions, assessments, and discharges, ensuring adherence to organizational policies.
  9. Take responsibility for the design, implementation, and documentation of treatment plans, ensuring alignment with the child’s needs and treatment objectives.
  10. Facilitate communication between treatment teams and external entities to address any external factors affecting treatment.
  11. Organize and lead meetings, case conferences, and assessments, ensuring comprehensive evaluation and revision of treatment plans.
  12. Represent the agency in custody hearings and other legal proceedings as necessary.
  13. In specific service areas, serve as the medication representative, coordinating with psychiatrists to optimize psychopharmacological treatment.
  14. Collaborate with management and teams to identify and cultivate new referral sources while strengthening existing relationships.
  15. Participate in training and professional development opportunities to stay informed about current trends in child and family treatment services.
  16. Monitor service delivery and collaborate with other providers to ensure cohesive treatment efforts.

Qualifications:

  1. Bachelor's degree in a relevant social science field or equivalent experience.
  2. Preferred: Case Management Certification.
  3. Must possess a valid driver's license.
