Job Title: Regional Family Nurse Practitioner

At Elevance Health, we are seeking a highly skilled and dedicated Regional Family Nurse Practitioner to join our team at Paragon Infusion Centers. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support to our patients in the clinical setting.

Job Summary:

The Regional Family Nurse Practitioner will be responsible for administering ordered therapies, completing admission processes, and performing physical assessments on patients. You will also be responsible for documenting patient data, reporting significant changes or observations, and communicating effectively with other team members.

Key Responsibilities:
  • Administer ordered therapies according to prescriber order, facility protocols, and company policy and procedures.
  • Complete admission process as directed by Center Director.
  • Perform physical assessment on each patient as warranted and for new consults as warranted by the patient diagnosis.
  • Refer to the NP on duty for complete physical assessment as needed.
  • Document all pertinent data in the patient's medical record.
  • Report all significant changes or observations to the Center Director and is responsible for the follow up on any problem which is identified.
  • Communicate effectively with other members of the IV team on patient status and observations.
  • Maintain patient confidentiality at all times by abiding by HIPAA laws and regulations.
  • Evaluate the patient's response to therapy and document this finding in the medical record.
  • Educate patients and/or family members regarding therapy plan.
  • Participate in the Quality Improvement program.
  • Ensure that all medication orders are complete, appropriate, accurate and up to date prior to treatment.
  • Participate in On-Call program, which may include after-hours, weekend and holiday calls and visitations.
  • Provide clinical coverage for nurse practitioners on PTO/LOA and may also support new market launches.
  • May require extensive travel to worksite and other locations as necessary.
Requirements:
  • Requires an MS in Nursing and minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
  • Experience with IV's required.
  • Current, active, valid and unrestricted NP license to practice as a healthcare professional within the scope of license in applicable state required.
  • Requires active, current and valid Family Nurse Practitioner Certification.
  • Multi-state licensure is required if this individual is providing services in multiple states.
  • Satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.
Preferred Skills, Capabilities and Experiences:
  • Minimum of 1 year healthcare experience with IV's strongly preferred.
Work Environment:

This position is based out of our Austin infusion center, TX-AUSTIN, 720 W 34TH ST, STE 104, and will be required to travel up to 75% to our 16 infusion centers across the Waco, San Antonio, and Houston areas.
