ADVANCED WOUND CARE NURSE PRACTITIONER

POSITION OVERVIEW: We are expanding our team of healthcare professionals and seeking dedicated Nurse Practitioners and Physician Assistants. The selected candidates will be responsible for managing patient consultations and conducting thorough examinations to ensure optimal health outcomes. Key responsibilities include performing diagnostic assessments to aid in patient care, maintaining precise and current patient documentation, and scheduling necessary follow-up visits. The ideal candidate will collaborate closely with our Physicians and Medical Assistants to provide exceptional care to all patients.

KEY RESPONSIBILITIES:
  • Conduct comprehensive physical assessments; perform initial screenings and document patient medical histories.
  • Carry out health evaluations, ensuring complete medical histories are recorded.
  • Document patient symptoms; update and verify patient records for accuracy during each visit.
  • Order and analyze laboratory tests and interpret results.
  • Formulate diagnoses; evaluate test outcomes and determine the necessity for follow-up care and additional treatment options.
  • Create individualized treatment plans that may incorporate relevant therapies.
  • Monitor patient progress to assess the effectiveness of prescribed treatments.
  • Deliver high-quality care while adhering to all organizational performance metrics.
  • Act as a vital resource for patients seeking ongoing care information, counseling, and support for complex health conditions.
  • Provide educational resources to patients to encourage health-promoting behaviors and disease prevention.
  • Engage in collaborative efforts with other healthcare professionals, including physicians and nursing staff.

REQUIREMENTS:
  • Required Degree: Family Nurse Practitioner (FNP) or Physician Assistant (PA).
  • Active Provider License in Texas.
  • Experience in Family Practice is preferred.

Employment Type: Full-time.

Compensation: $100,000 - $135,000 annually.

Medical Specialty: Primary Care.

Experience:
  • Minimum of 1 year as a Nurse Practitioner (Preferred).
  • Minimum of 1 year in Family Practice (Preferred).

License/Certification:
  • Texas Provider License (Required).
