Position Summary:

Under the guidance of the Population Health Manager, the Acute Care Operations Manager collaborates with healthcare providers, clinical support personnel, and other medical professionals to establish a comprehensive care model and enhance service delivery for patients and providers through a collaborative care approach. This role is crucial in managing daily priorities for patient services within a provider's panel, ensuring adherence to established quality metrics and clinical standards, including but not limited to Uniform Data System (UDS), Patient-Centered Medical Home (PCMH), and Healthcare Effectiveness Data and Information Set (HEDIS).

  • Proficiency in Microsoft Office Suite.
  • Understanding of quality improvement methodologies and experience with electronic medical record documentation.

Experience: A minimum of three years in a public health or outpatient care environment.

Education: High School diploma or GED; completion of a state-recognized nursing program and successful passage of the National Council Licensure Examination for Practical Nurses (NCLEX-PN).

Licensure/Certification: Valid nursing license in North Carolina (if applicable); current Basic Life Support (BLS) certification.

Key Responsibilities:

  • Evaluate data for assigned patient panels to identify trends, gaps, and opportunities for process enhancement.
  • Conduct pre-visit assessments to facilitate effective communication among care teams regarding necessary service delivery during patient encounters.
  • Maintain awareness of daily patient schedules to manage availability and triage walk-in patients.
  • Provide education and triage for walk-in patients based on established criteria.
  • Collaborate on quality improvement initiatives under the direction of the Practice Manager and designated Quality Improvement Champion.
  • Understand the roles of providers and support staff, as well as documentation standards, to effectively analyze clinical quality reports.
  • Facilitate new patient education on the Medical Home model, Patient Portal, and ancillary services.
  • Act as a liaison among patients, clinical teams, and various operational departments.
  • Oversee transitional care management and emergency department follow-ups for assigned panels.
  • Assist providers with managing electronic communications and related tasks.
  • Perform additional duties as assigned.
