Position Summary:
Under the direct oversight of the Population Health Manager, the Acute Care Manager collaborates with healthcare providers, clinical support personnel, and other professionals to establish a medical home and enhance care delivery for patients and providers through a Team-Based Care approach. The ACM plays a vital role in prioritizing daily interventions for patients assigned to a provider's panel, driven by established quality standards and clinical practices, including Uniform Data System (UDS), Patient-Centered Medical Home (PCMH), and Healthcare Effectiveness Data and Information Set (HEDIS).
Essential Qualifications:
- Exceptional verbal communication abilities.
- Ability to adapt to shifting priorities.
- Strong organizational, problem-solving, and analytical skills.
- Proficient in Microsoft Office Suite.
- Understanding of quality improvement methodologies and adeptness in electronic medical record documentation.
Experience: A minimum of three years in a public health or outpatient care environment.
Educational Background: High School diploma or GED, completion of a state-approved nursing program, and successful passage of the National Council Licensure Examination for Practical Nurses (NCLEX-PN).
Licensure/Certification: Unrestricted nursing license in North Carolina (if applicable); Current Basic Life Support (BLS) certification.
Key Responsibilities:
- Evaluate data for assigned patient panels to identify trends, gaps, and opportunities for process enhancement.
- Conduct pre-visit assessments to facilitate effective communication among care teams regarding service delivery during scheduled appointments.
- Maintain awareness of daily patient schedules to manage availability and triage walk-in patients.
- Provide education and triage for walk-in patients based on established evidence-based criteria.
- Coordinate quality improvement initiatives with guidance from the Practice Manager and assigned Quality Improvement Champion.
- Possess in-depth knowledge of provider roles, support staff functions, and EMR documentation standards to analyze clinical quality reports effectively.
- Facilitate educational sessions for new patients regarding the Medical Home model, Patient Portal, and ancillary services.
- Act as a liaison among patients, clinical teams, and various operational departments.
- Manage transitions of care and follow-up for assigned patient panels.
- Assist providers with inbox management and related tasks.
- Perform additional duties as assigned.
