Responsibilities- Visit patients in their homes, hospitals, or other settings, providing medical care and support as needed.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient information and progress in our electronic medical record system.
- Participate in medical staff meetings and other educational opportunities to stay current on best practices and industry developments.
- Current licensure as a Registered Nurse in the State of California.
- Current licensure as a Nurse Practitioner in the State of California.
- DEA certificate and valid National Provider Identification (NPI) number.
- Master of Science Degree in Nursing (MSN) from a recognized university.
- National Certification as a Nurse Practitioner (AANP or ANCC).
- Advanced Certification as a Hospice and Palliative Care Nurse (ACHPN) preferred.
- Minimum of three years of experience in hospice or palliative care.
