Job Summary

Key Responsibilities
  • Assess and develop individualized care plans for members, focusing on the most appropriate and least restrictive level of care.
  • Collaborate with members, families, and healthcare providers to ensure seamless care coordination and communication.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Provide education and support to members on community resources and benefits, empowering them to take control of their health.
  • Work closely with the care team to ensure timely and effective care delivery, addressing social determinants of health such as transportation, housing, and food access.
Requirements
  • Bachelor's degree in a health-related field or social work, or active LPN license.
  • At least 1 year of field-based experience, with a strong understanding of community resources and values.
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
  • Valid driver's license, current automobile insurance, and reliable transportation.
  • Ability to travel locally, up to 100 miles round trip, and up to 50% of the time.
Preferred Qualifications
  • Community Health Worker (CHW) accreditation.
  • Experience working in managed care, with knowledge of Medicaid/Medicare populations.
