Bilingual Case Manager

We are seeking a highly motivated and compassionate Bilingual Case Manager - Community Health Worker to join our team at the Community Renewal Team (CRT). As a key member of our Supportive Housing department, you will play a vital role in providing support and services to individuals in need.
Key Responsibilities- Provide outreach and engagement services to customers in the community, identifying those in need of case management and linking them with available services.
- Conduct initial interviews and assessments to determine program eligibility and develop individualized service plans (ISPs) that address customer needs.
- Facilitate referrals to clinics and community support services, coordinate services between providers and customers, and monitor customer compliance with ISP goals and objectives.
- Provide support, advocacy, and mediation services to ensure customers' overall needs and referrals are being conveyed.
- Document all encounters and interactions with customers, including activities, ISPs, and outcomes achieved.
- Conduct monthly home visits and bi-weekly conversations with customers to monitor progress and provide support.
- Maintain client records in accordance with documentation requirements and standards.
- Bachelor's degree from an accredited college or university required; education may be substituted with experience on a year-by-year basis.
- Two years of experience providing direct client services to populations experiencing substance abuse, homelessness, and/or mental illness required.
- Bilingual: Spanish written and verbal required.
- Certifications: Must have a CT Community Health Worker certificate or be able to obtain the certificate within six months of taking the position required.
- Knowledge of working with the chronically homeless population, Housing First, and Harm Reduction Intervention strategies.
- Ability to work with individuals from diverse racial/ethnic and economic backgrounds.
- Ability to offer strength-based case management and communicate effectively with customers, funders, and vendors.
