Job Title: Patient Navigator

Sun River Health is seeking a dedicated Patient Navigator to support our Southampton site. As a key member of our team, you will play a vital role in linking patients to essential healthcare services, ensuring they receive the care they need to thrive.

Key Responsibilities:
  • Collaborate with medical staff to identify patients in need of preventive and primary healthcare services.
  • Facilitate patient appointments within Sun River Health and outside agencies, following established procedures and preferred provider lists.
  • Assist patients in overcoming obstacles to receiving necessary referrals, ensuring seamless access to care.
  • Track progress and alert providers to facilitate timely referrals to specialty care and public health insurance programs.
  • Provide clear guidance to patients on provider instructions, procedures, and referral needs.
  • Transmit clinical information to specialists, obtain consultant reports, and obtain consent for information release.
  • Review open referrals and provide updates to the clinical team.
Requirements:
  • High School Diploma/GED required.
  • 2-4 years of related work experience.
