Patient Care Management: This position involves guiding patients through the entire care continuum, ensuring optimal resource utilization, service delivery, and adherence to external review standards.

Comprehensive Support: The Clinical Care Coordinator II provides continuous support and expertise through thorough assessments, care planning, implementation of plans, and overall evaluation of individual patient requirements.

Quality Enhancement: The role aims to improve patient management and satisfaction, promoting continuity of care and cost efficiency by integrating case management, utilization review, and discharge planning functions.

Care Coordination Responsibilities:

  • Oversees all aspects of patient care to ensure effective coordination and facilitation.
  • Maintains up-to-date knowledge regarding reimbursement methods, community resources, and relevant psychosocial and legal issues impacting patients and care providers.
  • Identifies and refers high-risk patients who may benefit from additional assistance.
  • Exhibits understanding of growth and development principles across the lifespan, providing age-appropriate care.
  • Engages in interdisciplinary rounds and conferences to assess treatment objectives, optimize resource use, educate families, and identify post-hospital needs.
  • Collaborates with the interdisciplinary care team, physicians, patients, families, post-acute providers, payors, and others to plan patient care throughout the continuum.

Knowledge Requirements:

  • Familiarity with government and non-government payer practices, regulations, standards, and reimbursement processes.
  • Understanding of Medicare benefits and insurance procedures.
  • Ability to apply critical thinking, prioritize tasks, and manage time effectively.
  • Proficient in basic computer skills, including Microsoft Office and spreadsheet software.
  • Excellent interpersonal, verbal, and written communication skills to foster collaboration with hospital leadership, physicians, payors, and external stakeholders.
  • Demonstrates strong interpersonal skills when interacting with patients, families, and colleagues.
  • Must possess the ability to read, write, and speak fluent English.
  • Graduation from an accredited nursing program is required. Licensure as a Registered Nurse or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) is necessary as per state regulations.
  • Certification in Case Management is advantageous.

Experience:
Two years of experience in a healthcare environment is preferred, particularly in case management, utilization review, or discharge planning.
