Role Description:

We are pleased to announce an opening for a Licensed Medical Provider (LMP) to join our Intensive Case Management (ICM) Team. The LMP will be responsible for conducting psychiatric evaluations and managing medication for clients engaged in ICM services at Cascadia. This role supports a Trauma-Informed Care framework and caters to individuals aged 18 and older diagnosed with serious mental health conditions. The ICM team offers intensive, adaptable community support aimed at improving clients' health outcomes by mitigating psychiatric symptoms and substance use, while enhancing their community involvement and overall quality of life. Due to the specific care needs of our clients, the ICM team maintains a lower staff-to-client ratio.

Key Responsibilities:
  • Assess medical necessity and appropriateness of mental health services for enrolled clients.
  • Document medical oversight by reviewing and signing comprehensive mental health assessments and service plans.
  • Conduct psychiatric evaluations for clients enrolled in the program.
  • Participate in daily ICM team meetings.
  • Evaluate clients' medical needs, including necessary lab tests.
  • Provide medication management and consultations, including prescribing and ongoing assessment of treatment efficacy.
  • Collaborate with Cascadia clinicians during treatment team meetings.
  • Maintain thorough and timely documentation of client care in accordance with agency standards.
  • Work collaboratively with other treatment team members and external providers.
  • Assist in crisis situations, adhering to emergency protocols.
  • Comply with mandatory reporting laws and HIPAA regulations.
  • Complete required training and participate in professional development opportunities.
  • Attend all scheduled meetings and supervision sessions.
  • Perform additional duties as assigned.

Qualifications:
  • For PMHNP: Must possess an active state nursing license as a PMHNP or DNP, certification from the American Nurses Credentialing Center, and DEA certification.
  • For Psychiatrist or DO: Must hold an active state medical license, DEA certification, and be board certified or eligible for certification by the American Board of Psychiatry and Neurology.
  • Preferred 2-5 years of clinical experience in a community mental health setting.
  • Proficient computer skills for timely documentation in electronic medical records.
  • Ability to provide care consistent with collaborative care principles.

Working Conditions:
  • Ability to navigate the clinic environment safely.
  • Occasional physical activities such as lifting (20 to 50 lbs), climbing, and running.
  • Regular sitting, standing, walking, and keyboarding.
  • Constant visual and auditory tasks.
