Role Summary:

We are pleased to announce an opportunity for a Licensed Medical Provider (LMP) to become a vital member of our Intensive Case Management (ICM) Team. The LMP will be responsible for conducting psychiatric evaluations and providing ongoing medication management for clients receiving ICM services, all while fostering a Trauma-Informed Care atmosphere. The ICM team focuses on individuals aged 18 and older diagnosed with serious mental health conditions, offering intensive, adaptable community support aimed at enhancing health outcomes by alleviating psychiatric symptoms and substance use, while promoting community involvement and quality of life.

Key Responsibilities:
  • Assess medical necessity and suitability of mental health services for enrolled clients.
  • Document medical oversight by signing off on comprehensive mental health evaluations, updates, and individual support plans.
  • Conduct psychiatric evaluations for enrolled clients.
  • Participate in daily ICM team meetings.
  • Evaluate clients' medical needs, including laboratory tests.
  • Provide medication consultations and management, including prescribing and ongoing assessment of medication effectiveness and safety.
  • Offer case consultations to fellow clinicians, including participation in regular treatment team discussions.
  • Maintain precise and current documentation of client care, adhering to agency standards for clinical and billing purposes.
  • Collaborate effectively with other treatment team members and external providers.
  • Assist in crisis situations as necessary, following established emergency protocols and coordinating with the treatment team.
  • Comply with mandatory reporting laws and HIPAA regulations.
  • Deliver timely, accurate, and professional documentation.
  • Complete annual training requirements and engage in seminars and educational opportunities.
  • Attend all scheduled staff meetings and supervision sessions.
  • Perform additional duties as assigned.
Qualifications:
  • For PMHNP: Must possess an active state nursing license as a PMHNP or DNP, certification from the American Nurses Credentialing Center, and DEA certification.
  • For Psychiatrist or DO: Must hold an active state medical license, DEA certification, and certification (or eligibility for certification) by the American Board of Psychiatry and Neurology.
  • 2-5 years of clinical experience in a community mental health setting is preferred but not mandatory.
  • Proficient computer skills for timely documentation in an electronic medical records system.
  • Ability to provide care in alignment with collaborative care principles informed by stages of change models.
Working Environment:
  • Must be capable of navigating the clinic environment safely, including assisting clients between waiting areas and meeting spaces.
  • Occasional physical activities such as stooping, lifting (20 to 50 lbs), climbing stairs, crouching, reaching, and running.
  • Regular activities include sitting, standing, walking, and keyboarding.
  • Constant visual and auditory engagement.
