**Job Summary**

We are looking for a seasoned RN with experience in case management, disease management, managed care, or medical or behavioral health settings. Your extensive knowledge of behavioral health and SUD conditions will enable you to effectively support our members. This is a remote position with 60% field travel within Vancouver. You will work Monday to Friday, 8:00 AM to 5:00 PM PST, developing and implementing comprehensive care plans tailored to the unique needs of our members. As a vital part of our interdisciplinary care team, you will collaborate closely with healthcare professionals to ensure seamless care coordination and outcomes-driven results.

Responsibilities:

  • Conduct comprehensive assessments of members per regulated timelines to determine eligibility for case management based on clinical judgment, changes in their health or psychosocial wellness, and triggers identified in the assessment.
  • Develop and implement individualized care plans in collaboration with members, caregivers, physicians, and other healthcare professionals to address their specific needs and goals.
  • Perform ongoing monitoring of care plans to evaluate effectiveness, document interventions, and suggest adjustments as necessary.
  • Maintain an active caseload for regular outreach and management to promote continuity of care and optimize member outcomes.
  • Promote integration of services for members, including behavioral health care and long-term services and supports/home and community-based services, to enhance care coordination and delivery.
  • Facilitate interdisciplinary care team meetings and informal ICT collaboration to foster effective communication and cooperation among team members.
  • Utilize motivational interviewing and Molina clinical guideposts to educate, support, and motivate positive change during member contacts.
  • Assess barriers to care, provide care coordination, and assistance to members to address concerns and improve overall well-being.

Requirements:

  • Bachelor's Degree in Nursing preferred; graduate from an Accredited School of Nursing required.
  • 1-3 years of experience in case management, disease management, managed care, or medical or behavioral health settings.
  • Active, unrestricted State Registered Nursing (RN) license in good standing; WA state RN license required.
  • Valid driver's license with a good driving record and reliable transportation.
  • Preferred: Bachelor's Degree in Nursing, Certified Case Manager (CCM), 3-5 years of experience in case management, disease management, managed care, or medical or behavioral health settings, and Behavioral Health experience.
