Job Description

We are seeking a highly skilled Registered Nurse to join our team as a Mental Health Case Manager RN.

DESC is a nonprofit organization dedicated to providing comprehensive services, treatment, and housing for individuals with complex needs related to homelessness, substance use disorders, and serious mental illness. Our vision is a community where no person is abandoned, ignored, or experiencing homelessness.

Job Summary

The successful candidate will work collaboratively with a multidisciplinary team to provide case management services to homeless and formerly homeless adults. This role requires strong clinical skills, effective communication, and the ability to work with diverse populations.

Main Responsibilities:
  • Patient Care and Support: Provide on-going assessment and care planning for clients, ensuring their physical and emotional needs are met.
  • Case Management: Coordinate medical aspects of care, including medication management, health education, and linkages to community resources.
  • Clinical Collaboration: Work closely with psychiatric staff, social workers, and other healthcare professionals to ensure comprehensive care.
Requirements:
  • Licensure: Current registration as a Registered Nurse in the State of Washington.
  • Clinical Expertise: Knowledge of psychiatric medications, side effects, and methods of administration.
  • Communication Skills: Effective verbal and written communication with clients, families, and healthcare teams.
  • Flexibility: Willingness to adapt to changing circumstances and priorities within the team.
