Key Responsibilities:
  • Identify and address quality care gaps to ensure optimal patient outcomes
  • Develop and implement effective care plans to improve patient health and well-being
  • Collaborate with healthcare teams to provide comprehensive care and support
  • Stay up-to-date with the latest clinical guidelines and best practices to ensure evidence-based care
Requirements:
  • Advanced degree as a Nurse Practitioner (ARNP or DNP)
  • Current and unrestricted license to practice medicine in Oregon State
  • Current and unrestricted DEA certificate
  • Current license to operate a motor vehicle and a reliable vehicle
Preferred Qualifications:
  • Experience as a primary care clinician
  • Experience in chart review and documentation inside an Electronic Medical Record (EMR)
  • Experience with geriatric patients in value-based care contracts such as Medicare Advantage
