Patient Transition Clinical Educator

Position Summary:
The Clinical Transition Educator plays a pivotal role in delivering clinical education and training, ensuring a seamless transition for patients into the Option Care Health delivery model. This position collaborates closely with the sales team to boost the number of patients transitioning to our care services.
Key Responsibilities:
- Assess, educate, and train patients, caregivers, and facility personnel on the integration of Option Care Health services and products in various settings, including in-home and virtual environments.
- Conduct thorough patient evaluations to ascertain their suitability for the Option Care Health delivery model.
- Work in partnership with the sales team to identify educational needs and implement strategies to enhance patient transition rates.
- Uphold the confidentiality of patient and proprietary information, adhering to legal standards for safeguarding sensitive data.
- Document educational activities accurately using Option Care Health's technology and applications.
- Must possess an active and unrestricted Registered Nurse (RN) license.
- A minimum of two years of experience in the healthcare sector is required.
- Proven ability to build and maintain relationships across various organizational levels and with external partners.
- Familiarity with standard practices and relevant regulatory standards in healthcare.
- Experience in delivering exceptional customer service and evaluating satisfaction levels.
- Basic proficiency in Microsoft Office Suite and other relevant software.
- Ability to identify operational challenges and propose effective solutions.
- Access to reliable transportation for local travel to care facilities and hospitals.
Local travel is essential to facilitate partnerships with healthcare facilities, community hospitals, and medical practices, providing live education and training support.
Preferred Qualifications:
- Experience in home or alternate site infusion therapy and discharge planning.
- Background in developing service provider partnerships.
