Job Summary

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team at Cottonwood Rehabilitation and Healthcare Center. As a LPN, you will be responsible for providing high-quality nursing care to our residents, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Provide nursing care to residents in accordance with their individualized care plans and state/federal regulations.
  • Participate in pre-admission assessments and develop comprehensive care plans to meet the unique needs of each resident.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Administer medications and treatments as ordered by physicians, ensuring accuracy and attention to detail.
  • Monitor resident vital signs, report changes, and implement interventions as necessary.
  • Assist residents with daily living activities, such as bathing, dressing, and grooming.
  • Document resident progress, including medical records and care plans.
  • Participate in quality improvement initiatives and contribute to a culture of excellence.
  • Maintain confidentiality and adhere to HIPAA guidelines.
Requirements
  • Graduation from an accredited School of Nursing.
  • Current license in the state of Illinois.
  • Current CPR certification.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Flexibility to work varied shifts, including weekends and holidays.
Work Environment

The LPN will work primarily in a skilled nursing facility, with occasional opportunities to work in other areas of the healthcare center. The work environment is fast-paced and dynamic, requiring the ability to multitask and prioritize tasks effectively.

The LPN will be required to stand, walk, and move freely throughout the facility, with occasional lifting and bending. The noise level is moderate, and the employee will be exposed to various medical equipment and supplies.
