Key Responsibilities- Provide nursing care to residents in accordance with their individualized care plans and state/federal regulations.
- Participate in pre-admission assessments and develop care plans that meet the unique needs of each resident.
- Collaborate with the healthcare team to ensure seamless care delivery and effective communication.
- Administer medications and treatments as ordered by physicians, ensuring accuracy and safety.
- Monitor resident conditions, report changes to physicians and supervisors, and implement interventions as needed.
- Support residents with daily living activities, such as bathing, dressing, and feeding.
- Maintain accurate and detailed records of resident care, including progress notes and medication administration.
- Participate in staff development programs to enhance skills and knowledge.
- Contribute to a positive and supportive work environment, promoting teamwork and excellent customer service.
- Graduation from an accredited School of Nursing.
- Current license in the state of Illinois.
- Current CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
- Flexibility to work varied shifts, including weekends and holidays.
The LPN will work primarily in a skilled nursing facility, with occasional opportunities to work in other areas of the healthcare center. The work environment is fast-paced and dynamic, requiring the ability to multitask and prioritize tasks effectively.
The LPN will be required to stand and walk for extended periods, use hands to finger, reach, and talk or hear. The employee may be required to lift and/or move up to 50 pounds independently and up to 200 pounds with assistance.
