Job Summary

We are seeking a skilled Case Manager to join our team at A Reputable Company. The successful candidate will be responsible for delivering comprehensive case management and program services for an assigned caseload of homeless families enrolled in the Family Crisis Housing Program.

The Case Manager will work under the direction of the Family Interim Housing Program Manager to provide an array of services and support that will enable each participant to become self-sufficient and capable of achieving permanent housing.

Key Responsibilities
  • Engage with families in crisis housing and complete intakes/enrollments for new participants
  • Liaise between Crisis Housing Time Limited Subsidy, Problem Solving, CES Matching, Housing Navigation & Family Advocate components of the Family Solution Center (FSC) to ensure that families receive appropriate services
  • Maintain confidentiality in all matters pertaining to the participants and their families
  • Develop effective and trusting professional relationships with participants to facilitate provision of services
  • Maintain a comprehensive caseload of participants and conduct timely follow-ups with head of household to develop housing plans
  • Assist in making referrals on behalf of the families in crisis housing and prioritize their needs
  • Respond to participants' concerns, complaints, and inquiries in a timely manner
  • Assist families with obtaining necessary program documents and connecting them with mainstream benefits and employment/training opportunities
  • Refer participants to appropriate services and facilitate connection to FSC co-located staff
  • Transport participants to various appointments, unit viewings, and events as needed
Requirements
  • From an accredited or state-approved college or university OR two years' experience working in the social service field, with case management and homeless program experience preferred
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds
  • Ability to communicate effectively, both written and orally
  • Verification of Employment Eligibility and Background Clearance
  • TB test required, CPR and First Aid Certification required within 30 days of employment, and valid Driver's License and auto insurance required
