Job Summary

Prime Healthcare is seeking a skilled Social Worker II to join our team at Centinela Hospital Medical Center. As a key member of our multidisciplinary team, you will provide emotional support, crisis intervention, and counseling services to patients and their families. Your expertise will help individuals cope with a range of social and emotional challenges, from pediatric to geriatric care.

Responsibilities

Provide case management and milieu management services to patients and families
Conduct psychosocial assessments and develop treatment plans
Collaborate with healthcare teams to coordinate discharge planning
Maintain accurate clinical records and documentation

Requirements

Master's degree in Social Work or related field
1+ year of hospital or outpatient experience in mental health care
Strong communication and interpersonal skills
Ability to work with diverse populations and complex cases
