Case Manager

Job Summary:
Exceptional Children's Foundation is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will provide essential support services to individuals with varying degrees of learning, behavioral, emotional, and developmental disabilities.
Key Responsibilities:
- Develop and maintain complete case records, notes, and other paperwork for all referrals and consumers.
- Collaborate with program staff to create individualized service plans that promote independence and goal achievement.
- Monitor consumer progress, provide crisis intervention as needed, and participate in team meetings and in-service training.
- Establish and maintain good working relationships with consumers, parents, and referral/service provider agencies.
- Adhere to ECF's Injury & Illness Prevention and Safety Program, and report work-related injuries/illnesses.
Requirements:
- BA in Human Service Field or related field.
- Two years of experience working with individuals with special needs in a direct client service capacity or one year as a Case Manager in a social service environment.
- Excellent verbal and written communication skills, basic math skills, and ability to work on multiple projects simultaneously.
- Valid California driver's license, reliable transportation, and insurable driving record.
- Pass a DOJ background investigation and provide evidence of a negative TB test.
Work Environment:
This position will be performed primarily indoors in a shared office environment. The employee may encounter consumers who are in crisis, exhibiting emotional/psychological distress, or behaving in an aggressive manner.
Compensation:
$20.00 Hourly to $24.00 Hourly
