Case Manager

As a Case Manager at Exceptional Children's Foundation, you will provide exceptional support to individuals with varying degrees of learning, behavioral, emotional, and developmental disabilities. Your primary responsibility will be to provide case management services, including case record keeping, advocacy, and service coordination. You will work closely with program staff to develop individual service plans that promote greater independence and monitor consumer progress. Additionally, you will assist in the development and implementation of behavior management programs, provide crisis intervention, and participate in team meetings and in-service training.
Responsibilities

• Develop individual service plans that promote greater independence
• Monitor consumer progress and work closely with staff on the implementation of goals and evaluation of each assigned consumer's progress
• Assist in the development and implementation of behavior management programs
• Provide crisis intervention as needed
• Participate in team meetings and in-service training
Requirements
• BA in Human Service Field
• Two years working with individuals who have special needs in a direct client service capacity or one year as a Case Manager in a social service environment
• Ability to work on multiple projects simultaneously
• Computer literate (MS Word and Excel)
• Valid California driver's license, reliable transportation, evidence of car insurance, and insurable driving record
• Bilingual, preferably in Spanish
Work Environment
This is a program for adults who have varying degrees of learning, behavioral, emotional, and developmental disabilities. Work will be performed primarily indoors in a shared office.
