Clinical Program Coordinator RN

Job Summary:
The Clinical Program Coordinator RN is a vital member of our team at Providence St. Joseph Health. This role requires a skilled and compassionate nurse to coordinate the care of behavioral health patients, working closely with physicians and other healthcare professionals to ensure consistent quality care.
Key Responsibilities:
- Coordinate the multi-disciplinary team responsible for patient care, providing education to patients and their families/guardians.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Evaluate, plan, and revise care as needed, adhering to hospital and nursing philosophy standards.
- Collaborate with healthcare teams to develop and implement care plans that meet the unique needs of each patient.
Requirements:
- Associate's Degree in Nursing or equivalent experience.
- Washington Registered Nurse License.
- 5 years of experience in an acute care setting.
Preferred Qualifications:
- Bachelor's Degree in Nursing or equivalent experience.
- 1 year of experience in a behavioral health setting.
