Position Overview

The Intake Nursing Coordinator is tasked with gathering initial and ongoing referral documentation from healthcare providers, insurers, and physicians to assess the suitability of services and/or facility placement. This role involves coordinating the creation of patient care plans that outline the frequency and duration of services, while also initiating care with service providers.

Essential Qualifications
  • High school diploma is mandatory
  • Current nursing license in the relevant state
  • A minimum of two (2) years of experience in healthcare administration at a managerial level is required
Key Responsibilities
  • Streamlines the inbound referral processes, documenting and reporting data related to admissions, discharges, and inquiries to senior management and facility administration.
  • Acts as a primary contact to verify insurance coverage, evaluate care needs, identify suitable facilities, and respond efficiently to hospital referrals.
  • Receives, updates, and engages with referrals, ensuring the organization can provide the appropriate level of care while gathering additional information for a seamless transition.
  • Assists patients and their families in identifying the most suitable services and support available in their community.
  • Collects data to facilitate the admissions process, ensuring all admission forms are accurately completed and signed.
  • Addresses challenges by establishing a singular access point for patients, families, and the organization, thereby enhancing follow-up procedures.
  • Reviews and audits the marketing information system to ensure all data elements are available for report generation, maintaining the integrity of the data entered into the system.
  • May temporarily take on additional responsibilities or assume a position in the event of an unplanned vacancy, provided qualifications are met.
  • Other duties and responsibilities may be assigned at any time, with or without notice.
