Job Title: Early Intervention Service Coordinator

Alta Mira NM is seeking a highly skilled and dedicated Early Intervention Service Coordinator to join our team. As a key member of our Early Intervention Program, you will play a vital role in supporting the development of young children with developmental delays or disabilities and their families.

Job Summary:

The Early Intervention Service Coordinator will be responsible for coordinating and facilitating evaluations, assessments, and services for children and their families. This includes conducting initial CMEs, drafting ECOs, and ensuring that services are delivered in accordance with the IFSP. The successful candidate will have excellent organizational, verbal, and interpersonal skills, as well as the ability to work effectively as a trans-disciplinary team member.

Responsibilities:
  • Intake and Orientation: Contact families within three business days of referral, complete orientation, and schedule CMEs.
  • Evaluation and Assessment: Complete initial CMEs, draft ECOs, and ensure that reports are clear and concise.
  • Individualized Family Service Plan (IFSP): Complete IFSPs within 45 days of referral, finalize ECOs, and ensure that on-going services start within 30 days.
  • Transition: Complete Part II of the transition plan, make referrals to appropriate agencies, and prepare assessment reports.
  • Discharge: Complete discharge process, including exit ECOs.
  • Other Responsibilities: Support the belief that a child's development is too complex to be addressed by a single discipline, follow program procedures, and complete other duties as assigned.
Requirements:
  • Prior Early Intervention experience preferred.
  • Knowledge of outside agencies, accessing SSI, Medicaid, etc.
  • BA/BS in social work, counseling, psychology, nursing, special education, Early Childhood education, or related field.
