Position Overview
This is a full-time remote position with working hours from 8 AM to 5 PM EST. As a leader in serving dual eligible populations, Aetna utilizes top-tier operational and clinical models to ensure quality care.

We work collaboratively with members, healthcare providers, and community organizations to address the comprehensive health care and social needs of our members. Our Care Managers serve as essential advocates for those who may not be able to advocate for themselves.

Key Responsibilities
- Conduct assessments, planning, implementation, and coordination of case management activities to evaluate members' medical needs and promote overall wellness.
- Develop proactive care plans that address identified issues to improve both short-term and long-term health outcomes.
- Utilize clinical tools and data analysis to assess members' needs and benefits.
- Apply clinical judgment to create strategies that mitigate risk factors and address complex health and social determinants affecting care planning.
- Engage in holistic assessments that consider various information sources, including claims data, to address all health conditions, including co-morbidities.
- Collaborate with interdisciplinary team members and supervisors to overcome barriers in achieving care goals.
- Employ motivational interviewing techniques to maximize member engagement and accurately assess their health status and needs.

Qualifications
- Active and unrestricted RN licensure in Florida or compact licensure in the state of residence.
- A minimum of 3 years of clinical experience, with at least 2 years in case management, discharge planning, or home health care coordination.

Preferred Skills
- Bilingual proficiency (English/Spanish).
- Certification as a Case Manager.
- Strong analytical and problem-solving abilities.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and proficiently navigate multiple systems and applications, including MS Office Suite.

Education Requirements
- Bachelor's degree or an Associate's degree with equivalent experience.

Compensation and Benefits
The typical pay range for this role is between $54,000 and $116,800 annually. This range reflects the base salary for all positions within this job grade. Actual salary offers will depend on various factors, including experience and education. In addition to competitive pay, CVS Health offers a comprehensive benefits package, including medical, dental, and vision coverage, a 401(k) retirement savings plan, and an Employee Stock Purchase Plan.
